- "Physician" refers to a licensed medical doctor employed by or contracted with Box of Drugs PLLC.
- "Medication(s)" refers to the prescription medications provided through the Box of Drugs program.
- "Telemedicine" refers to the provision of healthcare services using telecommunications technology.
1.3 Legal Capacity
You must be at least 18 years of age and possess the legal capacity to enter into these Terms. If you are accepting these Terms on behalf of another person, you represent and warrant that you have the authority to bind that person to these Terms.
2. Subscription and Payment Terms
2.1 Subscription Model
Box of Drugs PLLC operates on a subscription-based concierge model. Our services are not covered by insurance, and we do not submit claims to insurance companies or participate in any insurance networks.
2.2 Payment Terms
a) All services require upfront payment of the subscription fee.
b) Subscription fees are non-refundable except where prohibited by law.
c) Current subscription rates are published on our website and in your membership agreement.
d) We reserve the right to modify subscription rates with 30 days' notice to existing members.
2.3 Automatic Renewal
a) Your subscription will automatically renew at the end of your purchased subscription period unless canceled.
b) To cancel automatic renewal, you must provide written notice before the end of your current subscription period.
c) Cancellation requests received after the subscription period begins will be effective for the following subscription period.
d) We reserve the right to decline renewal of any subscription at our discretion.
2.4 Prescription Refills
a) Medications in your Box of Drugs will be automatically refilled based on reported usage and expiration dates.
b) You may opt out of automatic refills by providing written notice.
c) Lost or stolen medications will be refilled at the discretion of the physician and will be subject to regular refill rates.
2.5 Payment Methods
a) We accept major credit cards, debit cards, electronic funds transfer (EFT), PayPal, Venmo, and ApplePay.
b) By providing payment information, you authorize us to charge your chosen payment method for all fees incurred.
c) Failed payments may result in immediate suspension of services.
d) A fee of $35 will be assessed for checks returned due to insufficient funds.
2.6 Late or Missed Payments
a) Services may be suspended if payment is not received by the first day of the service month.
b) A grace period of 3 business days may be provided before service suspension.
c) In the event your subscription lapses, you may still call for consultation with the understanding that your subscription will be automatically restarted at the prevailing rate.
2.7 Financial Responsibility
a) You acknowledge that you are financially responsible for all charges associated with your subscription.
b) You agree to maintain current and valid payment information with us at all times.
c) Changes to payment information must be submitted at least 5 business days before the next billing date.
3. Box of Drugs Program Details
3.1 Program Overview
a) The Box of Drugs program provides members with a curated selection of prescription medications for potential emergency use.
b) All medications are provided under the supervision of licensed physicians.
c) Medications are prescribed and dispensed in accordance with all applicable state and federal laws.
3.2 Physician Consultation Requirements
a) Members MUST consult with an on-call physician before using any medication from the Box of Drugs.
b) Physicians will provide specific instructions for medication use during consultation.
c) Members agree to follow all physician instructions regarding medication usage.
d) Use of medications without prior physician consultation constitutes a violation of these Terms and may result in immediate termination of membership.
3.3 Medication Safety and Storage
a) Members must store all medications:
- At room temperature (68-77°F/20-25°C) unless otherwise specified
- In a dry place away from direct sunlight
- Out of reach of children and pets
- In their original containers with labels intact
b) Members must not store medications in bathrooms or other areas prone to temperature and humidity changes.
c) Members are responsible for maintaining the security of their Box of Drugs.
3.4 Usage and Reporting
a) Members must report all medication usage to Box of Drugs PLLC within 24 hours of use.
b) Members must report any adverse reactions immediately to Box of Drugs PLLC and seek appropriate emergency care if needed.
c) Members must maintain an accurate record of all medications used.
d) Regular inventory checks may be required as part of the program.
3.5 Travel Considerations
a) Members are responsible for checking and complying with all travel regulations regarding prescription medications.
b) Members should consult with Box of Drugs PLLC before traveling with their medications.
c) International travel with Box of Drugs medications requires prior consultation and documentation.
3.6 Medication Expiration and Disposal
a) Members must regularly check medication expiration dates.
b) Members must not use expired medications.
c) Box of Drugs PLLC will provide instructions for proper disposal of expired medications.
d) Members must not dispose of medications in household trash or flush them down toilets unless specifically instructed to do so.
3.7 Prohibited Actions
Members must NOT:
a) Share or sell medications from their Box of Drugs
b) Transfer medications to different containers
c) Allow others to access or use their Box of Drugs medications
d) Modify or alter medication packaging or labels
4. Telemedicine Services
4.1 Service Description
a) Box of Drugs PLLC provides telemedicine consultations via phone, video, or other electronic means as part of the subscription service.
b) Consultations are available 24 hours a day, 7 days a week for active members.
c) All telemedicine services are provided by licensed physicians in accordance with applicable state and federal laws.
4.2 Consultation Process
a) Members may initiate consultations through our designated communication channels.
b) Members must provide accurate and complete information about their medical condition during consultations.
c) Physicians will determine the appropriate course of treatment based on the information provided.
d) Members must disclose all medications they are taking, including those not provided through Box of Drugs PLLC.
4.3 Service Limitations
a) Telemedicine consultations have inherent limitations, including the inability to perform physical examinations.
b) Not all medical conditions are appropriate for telemedicine evaluation.
c) Our physicians reserve the right to require in-person evaluation when deemed necessary for proper diagnosis and treatment.
d) Emergency medical conditions may require evaluation at an emergency department or urgent care facility.
4.4 Emergency Situations
a) Telemedicine services are not a substitute for emergency medical care.
b) Members experiencing life-threatening emergencies should call 911 or seek immediate emergency care.
c) Our physicians will help direct members to appropriate care settings when needed.
d) Members agree to follow physician recommendations regarding emergency care.
4.5 Documentation
a) All telemedicine consultations will be documented in the member's medical record.
b) Members may request copies of their medical records in accordance with applicable laws.
c) Consultation notes may be shared with other healthcare providers when authorized by the member.
d) Records are maintained in compliance with HIPAA and other applicable regulations.
4.6 Technical Requirements
a) Members are responsible for having appropriate technology to participate in telemedicine consultations.
b) Members must ensure adequate internet connectivity for video consultations when required.
c) Technical difficulties during consultations may necessitate rescheduling or alternative communication methods.
4.7 Quality of Care
a) All telemedicine services are provided according to applicable standards of medical care.
b) All physicians at Box of Drugs are Board-Certified Emergency Medicine Physicians
c) Quality assurance reviews may be conducted to ensure appropriate care standards are maintained.
5. Patient Rights and Responsibilities
5.1 Patient Rights
Members have the right to:
a) Receive professional medical services with dignity and respect
b) Have their medical information kept confidential in accordance with HIPAA regulations
c) Participate in decisions regarding their medical care
d) Receive clear explanations about their treatment options and medications
e) Request copies of their medical records
f) Decline recommended treatments or medications
g) Know the names and qualifications of their healthcare providers
h) Be informed of any service changes that may affect their care
5.2 Patient Responsibilities
Members are responsible for:
a) Providing accurate and complete medical information

e) Keeping scheduled appointments or providing timely notice of cancellation

b) Contact Box of Drugs PLLC after emergency services have been activated
c) Follow all emergency care instructions provided by physicians
d) Provide follow-up information about emergency care received
7.3 After-Hours Care
a) Physician consultation is available 24/7 for active members
b) After-hours consultations follow the same protocols as regular consultations
c) Members must clearly communicate the urgency of their medical situation

7.4 Use of Box of Drugs in Emergencies
a) Members must contact a physician before using any medication, even in emergencies

d) Emergency medication usage may trigger automatic refill of used medications
7.5 Documentation Requirements

7.6 Emergency Prevention

8. Termination and Cancellation

8.5 Reactivation
a) Cancelled memberships may be reactivated at prevailing rates
